If you need medical services, please have your physician complete and submit the Medical Information Form

Passengers requiring a medical certificate

  • Passengers who suffer from serious or infectious diseases
  • Passengers who need medical attention during the flight, such as oxygen, injections, etc.
  • Passengers unable to use regular seats
  • Passengers who have recently suffered an external injury or undergone surgery

Information on submitting a medical certificate

  • Download the Medical Information Form (MEDIF) and the Privacy Policy and Consent Form
  • Fill in the downloaded forms with your doctor and submit them within 10 days before the departure date

  • Oxygen service fee is per cylinder for one way of an itinerary
  • For First and Prestige Class seats on some aircraft, you may need to purchase an additional seat to use the service.
  • After boarding, unused oxygen cylinders are non-refundable. 
  • If the currency of the departure country is not USD, CAD, or IDR, the USD fare exchanged for the local currency is charged. For further details, please contact the Service Center.

Instruction for Use

  • Passengers requiring the use of assistive breathing equipment are advised to contact the service center at least 48 hours prior to departure to confirm the maximum expected flight time.
  • For safety reasons, you cannot use the onboard electrical power. Only equipment with dry cell or up to 160-Wh lithium batteries may be used on board.
  • Please make sure you have enough battery capacity for at least 150% of your total flight time. 
  • Portable oxygen concentrators must have a Federal Aviation Administration (FAA) approval label or otherwise be specified SFAR 106 to be used on board.
  • In accordance with the American FAA regulations, you may be restricted from sitting in the emergency exit row seats, seats that hinder the evacuation of other passengers, and seats facing the walls (bulkhead seats).
  • If you require special medical assistance during your flight or experience a serious change in your health condition, the airline may request a medical certificate or doctor’s note to confirm these facts.
